The Community Alternative Program for Disabled Adults (CAP/DA)
The Community Alternative Program for Children (CAP/C)

The CAP Programs are Medicaid programs which offer home and community based services to eligible Medicaid recipients who face institutional placement. CAP care for the elderly, disabled adults and medically high-risk children whose health, safety, and well-being can be assured in the home setting.

Adult Day Health Care
In-Home Aide Services
Waiver Supplies
Home Mobility Aids
Preparation & Delivery of Meals
Respite Care
Telephone Alert

CAP/C patients may be considered for:

CAP/C Nursing Services
CAP/C Personal Care Services
CAP/C Waiver Supplies
Home Mobility Aids
Respite Care
