The Bridge Program

The Bridge Program is designed to “bridge the gap” between Home Health and Hospice. It provides care for patients who require specialized care, but who may not be hospice-appropriate due to their desire to continue aggressive treatment. Care is provided by the hospice staff that is cross-trained in home health.

Bridge services include:

• Skilled Nursing
• Home Health Aide Services
• Counseling
• Physical Therapy
• Speech Therapy
• Occupational Therapy
• Medical Supplies
• High Tech Therapies, such as Infusion Therapy and Chemotherapy
• 24-Hour Availability
