The Bridge Program
The Bridge Program is designed to bridge the gap between Home Health and Hospice. It provides care for Home Health patients who require specialized care due to chemotherapy or radiation treatments. Patients may not be hospice appropriate due to life expectancy or their desire to continue aggressive treatment. Care is provided by the hospice staff that is cross-trained in home health.
